Our services are available to individuals who have received emergency care and may have a third-party liability claim. You must be at least 18 years of age or have a parent or legal guardian consent on your behalf to use our services.
4. No Doctor-Patient Relationship
Injury Care Team provides case management services and does NOT provide medical advice, diagnosis, or treatment. Our services are administrative and coordinative in nature. Always consult with qualified medical professionals for healthcare decisions.
5. Costs and Payment
Our case management services are provided at no upfront cost to you. We work within the insurance and liability framework to ensure services are appropriately covered. Any potential costs will be discussed transparently with you before incurring any charges.
6. Your Responsibilities
By using our services, you agree to:
- Provide accurate and complete information about your case
- Respond promptly to communications from your case manager
- Notify us of any changes in your contact information or case status
- Cooperate with reasonable requests for documentation or information
- Not provide false or misleading information
7. Privacy and Confidentiality
We handle your personal and medical information in accordance with our Privacy Policy and all applicable healthcare privacy laws, including HIPAA. Your information is kept strictly confidential and shared only as necessary for case coordination with your consent.
8. Communications
By providing your contact information, you consent to receive communications from us related to your case via phone, email, or text message. You may opt out of non-essential communications at any time by contacting your case manager.
9. Limitation of Liability
Injury Care Team shall not be liable for any indirect, incidental, special, consequential, or punitive damages resulting from your use of our services. Our total liability shall not exceed the amount paid by you for our services, if any.
10. No Warranties
Our services are provided "as is" without warranties of any kind. We do not guarantee specific outcomes from our case management services, including the results of insurance or liability claims.
11. Termination
Either party may terminate the service relationship at any time with written notice. We will make reasonable efforts to ensure a smooth transition of your case if the relationship is terminated.
12. Dispute Resolution
Any disputes arising from these terms or our services will first be resolved through good faith negotiation. If negotiation is unsuccessful, disputes will be submitted to binding arbitration in accordance with the rules of the American Arbitration Association.
13. Governing Law
These terms shall be governed by and construed in accordance with the laws of the State of Texas, without regard to conflict of law principles.
14. Changes to Terms
